Call for Nominations
“The Village” a Unit of Prakash Bandhu Foundation, promoted by UP Nurses is pleased to invite nominations for our Nursing Excellence Award 2024.
Who can be nominated?
- Qualified Nurses working in Uttar Pradesh (UP)
- Nursing Students currently studying in UP
Nomination Categories:
- Best Principal
- Best Vice Principal
- Best HOD (from all Six Specialties including Forensic Science)
- Best Tutor/ Clinical Instructor (one from B.Sc., one from Post B.Sc., and one from M.Sc.)
- Best CHO (One from each GNM, Post Basic B.Sc., B.Sc., and M.Sc.)
- Best Clinical Nursing Professional (One from Government Setups and One from Private Setups with further categories e.g. One from each GNM, Post Basic B.Sc., B.Sc., M.Sc.)
- Best ANM (One from Government Setups one from Private Setups)
- Best Student (one from Government Setups and one from Private Setups with one each from further categories e.g. ANM, GNM, Post Basic B.Sc, B.Sc., M.Sc., PhD pursuing)
Nomination Process:
Nominations are now open and will be accepted through the following Google Form until May 29th, 2024. The form can be accessed here.
The entry fee for the nomination is just Rs. 180. It can be paid through the UPI QR code below.
Selection Process:
A distinguished selection committee will review all nominations and shortlist three nominees for each award category.
Award Ceremony:
Shortlisted nominees will be invited to attend the Award Ceremony, which will be held on June 29th, 2024, at the Convention Hall, The Village, A Unit of Prakash Bandhu Foundation, Khandoli – Etmadpur Road, Baharampur.
Accompanying Guests:
Selected nominees may be accompanied by up to two family members. A fee of Rs. 1,000/- per person will apply to the nominee and their accompanying guests to cover the cost of the ceremony. The fee can be paid through the UPI QR code below.
We encourage you to do a self-nomination or nominate deserving nurses and nursing students who are making a positive impact on the healthcare sector in Uttar Pradesh.
